IMMUNIZATION REQUIREMENTS FOR ALL STUDENTS K-12
All students must be up-to-date on their immunizations. Please carefully review this information and follow the proper steps to make sure your child is ready for school.
Below is a list of ALL REQUIRED immunizations:
- Two Meninggococcal (MCV4) – one dose is required for 7th grade and the second dose is required at age 16-years-old as of September 1 each year.
- All students in K-12 will be requited to have TWO varicella (chickenpox) vaccinations, unless documentation by a medical provider (MD, APRN, or PA only) is provided. No parent notes are accepted. If a student does not have the medical documentation, they must have the shot.
- One dose Tdap required for ALL students who are 11-years-old by September 1 of each year.
- One Hepatitis A required for all kindergarten students. (Must be given on or after first birthday.)
- ALL students K-12 are required to have:
- Four DTaP and three polio immunizations (last dose given on or after fourth birthday)
- Three Hep B doses
- Two MMR doses
- All kindergarten students must have all required immunizations, including 4-year-old shots, and a complete physical that includes a review of systems.
