Documents Required for Enrollment:

Arkansas laws require the district to obtain the following documents from all new students enrolling in Camden Fairview. Please present documents at the time of enrollment.

1. PROOF OF RESIDENCY IN CAMDEN FAIRVIEW SCHOOL DISTRICT. This can be any utility bill or statement with parent/guardian’s name and address dated within the past 30 days.
2. BIRTH CERTIFICATE. Or other official documents attesting to the child’s age.
3. SOCIAL SECURITY CARD.
4. CURRENT SHOT RECORD.
5. KINDERGARTEN SCHOOL PHYSICAL. Kindergarten students must have proof of a current physical before school begins. Contact your child’s doctor to schedule an appointment if needed.
